MotherToBaby says it is not known whether buspirone changes the chance of birth defects. A prospective registry report found no major malformations among 72 infants exposed in the first trimester, but this small study cannot settle the risk. The BuSpar product label says controlled human pregnancy studies have not been done and use should be limited to cases where clearly needed. UK product information advises avoiding buspirone as a precaution, while the EMA's 2025 buspirone review record lists the outcome as Maintenance. Ask your provider about your situation.

MotherToBaby describes one newborn with poor growth, tremors, low muscle tone, low blood sugar, and feeding trouble after exposure to buspirone, other medicines, and cigarette smoke during pregnancy. The 2021 case report describes several medicines taken together, so it cannot show that buspirone caused the symptoms.

Buspirone — Breastfeeding
Research strength: 2 / 5
LactMed says limited data suggest doses up to 45 mg a day produce low levels in milk, but long-term use data are missing and another medicine may be preferred for a newborn or preterm infant. In a 2024 study of nine mothers, buspirone was below detection in all milk samples, its active metabolite was found at low levels, and no infant adverse effects were reported. MotherToBaby and LactMed describe a 3-week-old with seizure-like activity while the mother took buspirone, fluoxetine, and carbamazepine; if a medicine caused it, the case authors thought fluoxetine was more likely. The BuSpar product label advises avoiding breastfeeding use if clinically possible, while MotherToBaby says treatment benefits may outweigh possible risks. The EMA's 2025 review recorded a maintenance outcome. Ask your provider about your situation.

Pregnancy
A prospective registry series reported no major malformations among 72 infants after first-trimester buspirone exposure; 68 women were evaluable, and the authors described the findings as preliminary and the sample as small. This study focused on major malformations after first-trimester exposure and does not answer later-pregnancy outcomes. A 2021 newborn case report describes symptoms after prenatal exposure to buspirone with sertraline, trazodone, venlafaxine, and maternal nicotine; it cannot identify buspirone as the cause. A 2026 critical review also describes buspirone-specific prenatal data as limited, mostly small cohorts and preclinical models.
Breastfeeding
A 2024 study collected milk samples and health histories from nine mothers taking buspirone. Buspirone was below detection in the samples, the active metabolite 1-PP was measured, and estimated relative infant doses ranged from 0.21% to 2.17%; no adverse effects were reported in the exposed infants. LactMed describes a 3-week-old with seizure-like activity during maternal buspirone, fluoxetine, and carbamazepine use; the case authors thought fluoxetine was more likely if a medicine caused the reaction. LactMed notes that long-term use during breastfeeding has not been studied.
Buspirone hydrochloride is the active ingredient in BuSpar. The matching BuSpar tablet label lists colloidal silicon dioxide, lactose monohydrate, magnesium stearate, microcrystalline cellulose, and sodium starch glycolate as inactive ingredients.
What was tested
Human pregnancy evidence includes a prospective first-trimester case series with 68 evaluable women and 72 infants, plus an individual newborn report with several medication exposures. Animal reproduction studies in rats and rabbits used buspirone doses of about 30 times the maximum recommended human dose. The 2024 breastfeeding study measured buspirone and its active metabolite in milk from nine mothers and reported infant observations from caregiver health histories. LactMed says long-term breastfeeding-use data are unavailable.
Different product-label advice for buspirone during pregnancy
- Source A says
- The UK product information says, 'As a precautionary measure, it is preferable to avoid the use of buspirone during pregnancy.' (UK eMC: Buspirone Hydrochloride 10 mg Tablets Summary of Product Characteristics)
- Source B says
- The US BuSpar product label says use during pregnancy only if clearly needed. MotherToBaby says the effect on birth-defect risk is unknown, and Freeman et al. reported no malformations among 72 first-trimester-exposed infants. (DailyMed: BuSpar (buspirone hydrochloride) label, Direct Rx repackaged label, MotherToBaby: Buspirone (Buspar) fact sheet, Freeman et al., Pregnancy outcomes after first-trimester exposure to buspirone, 2022)
- Why they differ
- The product labels use different caution levels while both describe limited human pregnancy data. The US label is dated 2016, while the UK product information was updated in October 2026. The small first-trimester case series does not settle risk, and the difference in label wording is not evidence of an established harm.
- Other sources
- The EMA's buspirone PSUSA record lists the regulatory outcome as 'Maintenance'. (European Medicines Agency: Buspirone PSUSA/00000463/202404)
- What the evidence shows overall
- The available human pregnancy data remain limited. The first-trimester series observed no malformations, but its small sample cannot rule out an increase. The label difference is a real precaution disagreement; guidance is retained rather than an avoid rating based only on limited data.
Product-label caution and newer milk-transfer data during breastfeeding
- Source A says
- The BuSpar product label says human milk excretion is unknown and breastfeeding use should be avoided if clinically possible. LactMed says another medicine may be preferred for a newborn or preterm infant because long-term data are unavailable. (DailyMed: BuSpar (buspirone hydrochloride) label, Direct Rx repackaged label, LactMed: Buspirone)
- Source B says
- MotherToBaby says treatment benefits may outweigh possible risks. In a 2024 study of nine mothers, buspirone was below detection in milk samples, low levels of the active metabolite were found, and no adverse effects were reported in exposed infants. (MotherToBaby: Buspirone (Buspar) fact sheet, Krutsch et al., Alleviating anxiety while breastfeeding: evaluating buspirone transfer into human milk, 2024)
- Why they differ
- The US product label was updated in 2016 and described milk excretion as unknown. The 2024 study adds small-sample milk measurements but does not answer long-term infant questions, which helps explain why current summaries retain caution.
- Other sources
- The EMA's buspirone PSUSA record lists the regulatory outcome as 'Maintenance'. (European Medicines Agency: Buspirone PSUSA/00000463/202404)
- What the evidence shows overall
- The nine-mother study found low transfer into milk but does not establish long-term infant outcomes. Keep guidance, show the product-label caution and the newer milk study, and do not treat the small study as a complete answer.
- MotherToBaby says it is important to talk with healthcare providers before making changes to how the medication is taken. (MotherToBaby)
- MotherToBaby says that if someone plans to stop buspirone, their provider may suggest slowly lowering the dose instead of stopping all at once. (MotherToBaby)
- MotherToBaby says providers can discuss the benefits of treating the condition and the risks of untreated illness during pregnancy. (MotherToBaby)
